Single Service Desk (Full-Purpose Desk)
To respond to the “Implementation Project on the Single Administration Window for all Units of Executive Yuan” and help people handle issues, we have set up a single service desk at the entrance of the Administrative Building on May 1, 2012 to help people with all kinds of applications. We accept applications at one spot and help people throughout the entire process to reduce the operation, speed up administration, save the effort for going back and forth between application desks and enhance the convenience of services.

Services
1. Document application:
 (1) School leaving certificate/In-School certificate
(2) Various certificates of student status
(3) Official seal for power of attorney

1. Application for home visit/funeral attendance

1. Medical treatment for student:
   (1) Medicine delivery
   (2) Application for compassionate release
   ※Documents and information required:
    (1) Medicine delivery:
           Sealed medicine bag (sub-packaging not allowed), prescription, certificate of diagnosis (reason for

    Medication), guarantee for no replacement of medicine, ID and seal

   (may be replaced with signature) of the applicant (agent).
 (2) Compassionate release:
  A. General compassionate release: A family member brings the certificate of diagnosis (issued in 10 days), and the ID and seal of the applicant

   (Agent) to us to apply for general compassionate release. After the approval of the Agency of Corrections, Ministry of Justice,

    we notify the family member to the local district prosecutors office to complete the bail procedure and

    we will release the student.
  B. Emergency compassionate release: We will submit the certificate of diagnosis, critical condition notice

    and ID copy to the local district prosecutor’s office for approval.

    We refer the family member to the local district prosecutors’ office to complete the emergency power bail procedure and collect the student from our school (or

    Hospital). We will declare the release to the Agency of Corrections, Ministry of Justice.
